What you will do:

- Independently manage and prioritise a busy caseload, ensuring cases progress efficiently from referral to closure
- Screen and triage referrals, assess risk and urgency, and make sound professional decisions
- Complete functional and environmental assessments, moving and handling assessments, recommend equipment and minor adaptations, incorporating strengths-based, person-centred practice
- Collaborate closely with service users, carers, colleagues, and partner agencies to deliver comprehensive support
- Maintain accurate and detailed assessments, clinical reasoning, written communication, and case recording
- Apply your extensive knowledge of safeguarding adults, the Mental Capacity Act, Care Act, and relevant legislation
- Utilise electronic case management systems and Microsoft Office applications effectively
