What you will do:
- Undertake Occupational Therapy assessments in diverse community settings.
- Assess disability-related needs and recommend appropriate interventions.
- Recommend and arrange specialist equipment and adaptations to meet client needs.
- Advise on improving accessibility within home and community environments, ensuring clients can maximise independence.
- Support individuals to enhance their quality of life through rehabilitation-focused interventions and goal-based programmes.
- Provide advice, counselling, support, and training to individuals, families, and carers.
- Liaise with health, social care, and voluntary sector professionals to ensure comprehensive care delivery.
- Contribute to Integrated Community Equipment Service projects and support community rehabilitation programmes.
- Supervise support staff and delegate work, as well as participate in staff training and student education.
- Maintain accurate records, reports, and case documentation to ensure continuity of care.
Requirements:
- Occupational Therapy qualification and HCPC registration.
- Understanding of Occupational Therapy assessment processes and knowledge of relevant legislation.
- Experience with disability-related needs and environmental adaptations.
- Ability to work independently and as part of a team, managing a varied workload effectively.
- Strong communication, report-writing, and organisational skills.
- Commitment to equality and inclusive practice.
- Must meet the travel requirements of the role, including occupational driving.
