What you will do
- Undertake strengths-based assessments, reviews, and care planning for adults with complex mental health needs in accordance with the Care Act 2014.
- Manage a diverse caseload within community and hospital settings, ensuring timely interventions and effective risk management.
- Complete safeguarding enquiries and work to protect vulnerable adults from abuse or neglect.
- Collaborate with NHS professionals, Approved Mental Health Professionals (AMHPs), community mental health teams, housing providers, and voluntary sector partners.
- Participate in Mental Health Act assessments and contribute to statutory mental health functions when required.
Person Specification
Qualifications
- Degree or equivalent qualification in Social Work.
- Successful completion of Assessed and Supported Year in Employment (ASYE) desirable.
- Advanced post-qualifying development in adult mental health practice is advantageous.
Registration and Compliance
- Current registration with Social Work England (SWE).
Experience
- Experience working within Adult Social Care or Community Mental Health Services.
- Demonstrated ability to manage complex mental health cases and safeguarding concerns.
