What you will do
- Conduct comprehensive assessments to determine the care needs of adults with mental health concerns.
- Develop, implement, and review care plans in partnership with service users and their families.
- Liaise with external agencies and professionals to deliver coordinated care services.
- Provide support and guidance to individuals and their families, advocating for client needs.
- Monitor and evaluate progress, adjusting plans as necessary to ensure optimal outcomes.
- Fulfil case management duties, maintaining accurate and up-to-date documentation.
- Collaborate with interdisciplinary teams within the Mental Health Team to deliver targeted interventions.
