What you will do:
- Responsible for carrying out comprehensive assessments within the community for people who have either a permanent or a complex and substantial disability, facilitating service provision that promotes self-reliance and prevents the need for ongoing care.
- Conduct assessments under relevant legislation such as the Care Act and Housing Grants, Construction and Regeneration Act, ensuring care provision accommodates environmental changes that foster independence.
- Join a Multidisciplinary Team to review the effectiveness of current care and explore innovative support strategies aimed at enhancing user independence.
- Supervise and guide Occupational Therapy Assistants and students effectively, contributing to a supportive learning environment.
- Execute tasks within the OT and Sensory service to support Adult Social Care's mission and vision of promoting user independence and inclusion.
- Collaborate with service users, carers, and diverse professionals to create a supportive network focused on maximizing individual independence.
You must be registered with the HCPC. A valid driving licence is also essential as the role involves traveling within the community to conduct assessments and provide support.
