What you will do:
- Provide high-quality mental health care and interventions to adults within the community mental health team.
- Conduct comprehensive assessments to determine individual patient needs and develop tailored care plans.
- Deliver short-term interventions to support patient discharge and transition back into the community.
- Collaborate with a multidisciplinary team to ensure holistic care and treatment.
- Maintain accurate and timely patient records and documentation.
- Use your knowledge of care pathways to ensure effective patient flow and support.
Requirements:
- Must be NMC Registered and have experience as a mental health nurse.
- Previous experience in Adult CMHT and with mental health cases is essential.
- Knowledge and experience in delivering short-term interventions to support discharge.
- Familiarity with care pathways is beneficial for enhancing patient outcomes.
