What you will do:
- Assess, plan, and deliver specialist physiotherapy interventions tailored to individual patient goals.
- Support patients in developing independence with personal care, domestic tasks, functional mobility, and overcoming cognitive or sensory challenges.
- Use a range of therapeutic techniques and adaptive equipment to promote recovery and enhance quality of life.
- Manage a complex caseload independently, providing specialist input into treatment plans and discharge decisions.
- Complete risk assessments and home environment evaluations to support safe discharge planning.
- Liaise with community services, social care, and other agencies to ensure continuity of care.
The ideal candidate will need to be an HCPC registered practitioner and hold a valid UK driving licence for local travel between appointments.
