With a competitive rate of £36 hourly, this full-time locum role is ideal for those seeking flexibility and variety in their career. The team, currently located at Tolworth Hospital, works under a hybrid model, ensuring some days can be spent working from home. Plus, the Mental Health Social Care Team comes with its own charter and is supported by a Team Manager and Deputy Manager.

What you will do:
- Deliver duty cover for the Mental Health Social Care Team
- Perform AMHP duties if qualified
- Conduct Care Act Assessments and determine eligibility
- Undertake care planning and commissioning in collaboration with brokerage
- Engage in SAFEGUARDING investigations and provide professional social work support
- Participate in team meetings, allocations, and review supported living and residential care placements
- Foster recovery and independence through step-down in supported living environments
- Offer crisis intervention and social work support for those living independently, including appointeeship
