What you will do:
- Conduct comprehensive occupational therapy assessments, including functional, cognitive, and Activities of Daily Living (ADL) evaluations.
- Develop and implement patient-centred treatment plans tailored to individual needs and rehabilitation goals.
- Manage a diverse caseload of patients with complex physical, cognitive, and functional conditions.
- Facilitate safe, effective, and timely discharge planning in collaboration with the multidisciplinary team (MDT).
- Assess, recommend, and arrange appropriate equipment and minor adaptations to support patient independence and safety.
- Work closely with patients, families, ward staff, and community services to ensure continuity of care.
- Maintain accurate, up-to-date, and compliant clinical records and documentation.
- Prioritise workload effectively and manage caseload independently within a busy inpatient setting.
- Participate actively in multidisciplinary team meetings, case discussions, and discharge planning processes.
- Provide occupational therapy interventions across acute, rehabilitation, frailty, and community care pathways as required.
- Monitor patient progress, evaluate outcomes, and adjust treatment plans accordingly.
- Ensure clinical practice aligns with HCPC standards, professional guidelines, and Trust policies.
Requirements:
- HCPC Registration: HCPC registration as an Occupational Therapist.
- Qualification: Recognised Occupational Therapy qualification.
- Experience: Experience working within acute inpatient, rehabilitation, frailty, or community settings.
- Clinical Skills: Ability to manage complex patients autonomously and make sound clinical decisions.
- Communication Skills: Strong communication, teamwork, and organisational skills.
- Driving Licence: Must possess a valid driving licence, providing the capability to visit patients across different locations as needed.
