What you will do:

• Clinical Assessment & Prescribing: Conduct comprehensive assessments for individuals accessing substance misuse services and independently prescribe in line with clinical guidelines

• Care Planning & Reviews: Develop, monitor, and review personalised care plans, supporting service users to achieve recovery goals

• Substance Misuse & Harm Reduction: Provide harm reduction advice and support detoxification and stabilisation plans

• Physical & Mental Health Monitoring: Assess and monitor associated health conditions, facilitating access to primary and specialist services

• Safeguarding & Risk Management: Identify safeguarding concerns and manage risks to ensure service user safety

• Multi-Disciplinary & Partnership Working: Collaborate with internal teams and external partners to deliver holistic care

• Clinical Governance & Professional Practice: Maintain high standards of clinical record keeping and contribute to service improvement initiatives

Qualifications:

• Registered Nurse, Pharmacist, or Allied Health Professional with an Independent Prescriber qualification (V300)

• Current registration with NMC, GPhC, or HCPC, with evidence of continuing professional development and revalidation

Experience:

• Essential experience within substance misuse, community health, or mental health services, including independent prescribing

• Desirable knowledge of opioid substitution therapy and recovery-focused practice
