What you will do:

Provide a ward-based clinical pharmacy service to areas across the Trust ensuring the accuracy, legality, safety, and efficacy of prescribed medication
Ensure medicines management, reconciliation, and dispensing are undertaken in line with current medicines legislation
Conduct clinical screening and authorise homecare prescriptions, liaising with clinical teams to resolve problems and queries
Collaborate with the Pharmacy Technician Homecare Specialist to provide a comprehensive homecare service to Trust patients

To qualify for this role, you must be registered with the HCPC. While being a car driver is advantageous, this will support smooth travel between sites within the Trust and the efficient delivery of services.
