What You Will Do:

Deliver high-quality, person-centred care to individuals with mental health needs in community settings.
Develop, implement, and review Section 117 aftercare plans, ensuring compliance and continuity of care.
Conduct comprehensive assessments and tailor care plans to individual needs.
Collaborate with multidisciplinary teams and partner agencies to provide holistic support.
Maintain accurate and compliant clinical records.
Offer advice, education, and reassurance to families and carers throughout the care process.

About You:

Registered mental health professional (HCPC registered as specified).
Experience in S117 aftercare planning.
Full UK driving licence to support community-based visits.
Strong communication and risk assessment skills.
