What you will do:

Deliver specialist clinical pharmacy services to ensure optimal patient care.
Provide expert pharmaceutical advice as a key member of multidisciplinary teams.
Undertake medicines reconciliation and clinical screening, keeping patient safety a top priority.
Support medicines optimisation initiatives to improve overall medication management.
Review and monitor medication therapy plans for efficacy and safety.
Ensure the safe and effective use of medicines throughout the healthcare process.
Contribute to ward-based clinical pharmacy services and keep impeccable records.
Participate in patient safety and governance initiatives to uphold high care standards.

Requirements:

GPhC registration
Experience as a Band 8a Clinical Pharmacist with relevant UK NHS background
Hospital and clinical pharmacy experience
Strong knowledge of medicines optimisation
Ability to work independently with excellent communication skills
Experience within acute hospital services
Independent Prescribing qualification
Innovative mindset to support service development
