What you will do:
- Deliver specialist clinical pharmacy services to ensure optimal patient care.
- Provide expert pharmaceutical advice as a key member of multidisciplinary teams.
- Undertake medicines reconciliation and clinical screening, keeping patient safety a top priority.
- Support medicines optimisation initiatives to improve overall medication management.
- Review and monitor medication therapy plans for efficacy and safety.
- Ensure the safe and effective use of medicines throughout the healthcare process.
- Contribute to ward-based clinical pharmacy services and keep impeccable records.
- Participate in patient safety and governance initiatives to uphold high care standards.

Requirements:
- GPhC registration
- Experience as a Band 8a Clinical Pharmacist with relevant UK NHS background
- Hospital and clinical pharmacy experience
- Strong knowledge of medicines optimisation
- Ability to work independently with excellent communication skills
- Experience within acute hospital services
- Independent Prescribing qualification
- Innovative mindset to support service development
