We are seeking a dynamic Family Support Worker to join our dedicated Children with Disabilities Team in Ilford. What you will do
- Undertake and monitor reviews of children with disabilities, working to minimise the effects of their disability.
- Facilitate access to appropriate and beneficial services for families and children.
- Identify and refer child protection issues to Senior Practitioners or the Team Manager.
- Develop strong links with Health, Education, Connexions, and Adult and Children’s Social Services.
- Complete various assessments and prepare reports to support children’s well-being and care plans.
- Work collaboratively with parents and professionals in planning and delivering services.
- Maintain and update comprehensive records using specified electronic systems.
- Liaise with relevant professionals in assessing and planning services for children with disabilities.
- Work flexibly, including outside regular office hours and in various venues, to meet the diverse needs of children and families.
- Contribute to team meetings and service development with constructive input.
- Undertake the Duty Family Support Worker role and provide support within the Families Together Hub as required.
