What you will do:

• Provide expert intervention to enhance patient recovery and assist in hospital discharge processes.
• Apply your knowledge in intermediate care, discharge to assess and assessment frameworks to support patient transitions.
• Utilise SystmOne to maintain accurate and up-to-date patient records.
• Travel to various sites and patients' homes as required, ensuring high-quality care continuity.

Additional requirements:

• Registered with HCPC.
• Must possess a full driving license and access to a vehicle for travel.
