What you will do:

- Conduct comprehensive assessments of the needs of older adults and develop personalised care plans.
- Work collaboratively with team members and external agencies to deliver holistic support for service users.
- Offer guidance and support to individuals and their families, ensuring their social and emotional needs are met.
- Maintain accurate records and case notes, using ICT systems as needed to track and manage caseloads effectively.
- Stay informed of relevant legislation and best practices to maintain high professional standards in all interactions.
- Advocate for the rights and needs of older adults within the community, promoting independence and quality of life.
- Participate in regular team meetings and contribute to the ongoing improvement of team services and processes.
