What you will do:

- Conduct comprehensive assessments of patients' needs in various acute wards.
- Develop and implement effective treatment plans tailored to individual patients.
- Facilitate safe and efficient discharge planning in collaboration with community services.
- Support patient flow and manage discharge processes to reduce hospital stay times.
- Work five days over seven, potentially including weekends, to meet the demands of the service.

Requirements:

- HCPC registration
- Minimum of 2 years’ experience in acute hospital settings
- Strong communication and organisational skills
- Car driver preferred to accommodate potential community follow-ups
