What you will do:

  • Undertake Care Act assessments and Mental Capacity Act / Best Interests decisions to support safe and timely hospital discharges.

  • Safeguard vulnerable adults at risk of abuse or neglect, managing cases with appropriate legal and procedural authority.

  • Complete strengths-based assessments to determine eligibility and promote wellbeing and independence.

  • Carry out positive risk assessments, ensuring least restrictive interventions wherever possible.

  • Play a central role in adult safeguarding processes and enquiries.

  • Participate in the duty rota, contributing to shared team responsibilities.

  • Maintain accurate and up-to-date case records using council IT systems and procedures.

  • Work with complex cases while supporting and mentoring colleagues and students where appropriate.

  • Ensure compliance with legislative frameworks, statutory guidance, and council practice standards.

  • Collaborate effectively with multidisciplinary professionals, providing clear verbal and written reports.

  • Maintain continuous professional development (CPD) and uphold Social Work England (SWE) registration requirements.

Qualifications:

  • Recognised Social Work qualification with current Social Work England registration.

  • Proven experience working with vulnerable adults and carers.

  • Up-to-date knowledge of legislation, statutory guidance, and best practice.

  • Ability to apply social work methods to promote independence and wellbeing.

  • Strong IT skills to support accurate recording and reporting.

  • Enhanced DBS and barred list clearance (previous convictions assessed as part of recruitment).
