What you will do:

  • Collaborate with key partners in the Community Team for Learning Disabilities Health and Intensive Support Team.

  • Carry out person-led assessments for people aged 18 and over with learning disabilities.

  • Apply a strengths-based approach to promote positive change, independence, and self-determination.

  • Manage a complex caseload effectively, prioritising needs and assessing risks appropriately.

  • Undertake court work and support planning when required.

  • Travel across the Wakefield district to meet the needs of individuals.

  • Work in a person-centred way to make a genuine difference to people’s lives.
