What you will do:
- Undertake comprehensive nutritional assessments and devise effective treatment plans.
- Offer dietary advice and support to patients, relatives, and carers.
- Evaluate nutritional requirements, considering medical histories and social factors.
- Liaise with multidisciplinary team members to ensure seamless, coordinated care.
- Balance the risks and benefits of dietary interventions.
- Assist in weight restoration following unexpected weight loss.
- Provide initial gastroenterology and diabetes advice.
- Keep detailed clinical records and documentation.
- Engage in multidisciplinary team discussions for service improvement.
Requirements:
- Hold a Degree or Postgraduate Qualification in Dietetics with HCPC registration.
- Proven experience in Community Dietetics and remote working.
- Skilled in nutritional assessments and knowledgeable about gastroenterology and diabetes management.
- Self-motivated with a strong capacity for independent work.
- Excellent communication and report-writing expertise.
- Experience working within a multidisciplinary team.
Desirable Requirements:
- Previous experience in Adult Community Dietetics.
- Experience in chronic disease management.
