Job Title: Adults Advanced Practitioner within Hospital Discharge in Salford (UK)

We have an exciting opportunity for an Adults Advanced Practitioner to join the Hospital Discharge team at Salford Royal. This role offers a 12-month contract paying £32.55 hourly, operating Monday to Friday from 08:00 to 16:00. You will play a critical role in supporting hospital discharge processes, covering maternity leave with consistency and commitment that sets this position apart from typical locum roles.

- Liaise with community services to ensure comprehensive post-discharge care plans
- Maintain accurate records and reports as required by regulatory standards and hospital policies

Requirements of the Advanced Practitioner:
- Degree or equivalent in social work
- Current registration with Social Work England
- Significant frontline experience in Social Services within the UK, particularly in a hospital setting
