What you will do:

  • Work as an integral member of the Multi-Disciplinary Team (MDT), evaluating and coordinating social care delivery for adults and carers.
  • Engage with the Multi Agency Safeguarding Hub (MASH) to ensure the best safeguarding practices are followed.
  • Conduct comprehensive needs assessments and implement subsequent support plans in alignment with strength and asset-based practices.
  • Provide advice and guidance while advocating for the rights and entitlements of adults with complex health and mental health needs.
  • Lead investigations under the Care Act (2014) for adults at risk, ensuring timely and effective outcomes.
  • Communicate critical assessment information to relevant stakeholders, handling sensitive issues professionally.
  • Draft and submit funding applications as needed, collaborating effectively with local commissioners and health colleagues.
