What you will do:

• Undertake strengths-based Occupational Therapy assessments to identify needs, abilities, risks, and opportunities for greater independence.

• Develop and implement creative, person-centred interventions that support individuals to achieve their goals and maximise wellbeing.

• Assess functional abilities within home and community environments and recommend appropriate solutions.

• Provide advice, equipment recommendations, adaptations, and interventions that promote safety and independence.

• Work closely with adults, carers, families, and partner agencies to deliver coordinated support.

• Promote preventative approaches that reduce the need for long-term care and support services.

• Contribute to care planning, risk management, and outcome-focused interventions aligned with statutory responsibilities.

• Maintain accurate, timely, and comprehensive case records using electronic recording systems.

• Communicate effectively with colleagues, health professionals, housing providers, and other agencies to achieve positive outcomes.

• Support service improvement initiatives and contribute to the ongoing development of Occupational Therapy practice.

• Uphold the professional standards of the Royal College of Occupational Therapists (RCOT) and the Health and Care Professions Council (HCPC).

• Participate in supervision, training, and continuous professional development activities.

Requirements:

• Degree or equivalent qualification in Occupational Therapy.

• Current HCPC registration.

• Experience working within community-based Occupational Therapy, Adult Social Care, or related settings.

• Strong understanding of strengths-based practice, person-centred interventions, and relevant legislation.

• Excellent communication, assessment, and problem-solving skills.

• Full UK driving licence and access to a vehicle.

• Ability to work independently while contributing effectively to a wider multidisciplinary team.
