What you will do:

• Undertake comprehensive Occupational Therapy assessments focusing on functional ability, environmental barriers, and risk management.

• Develop and implement person-centred intervention plans that promote independence, wellbeing, and positive outcomes.

• Deliver early intervention and rehabilitation programmes aimed at preventing hospital admissions and reducing long-term care needs.

• Assess, recommend, and prescribe specialist equipment, assistive technology, and minor adaptations.

• Complete assessments for major adaptations and liaise with housing services, surveyors, and contractors to coordinate implementation.

• Undertake moving and handling assessments and provide guidance, recommendations, and training to individuals and carers.

• Carry out risk assessments and develop balanced risk management plans that support independence while ensuring safety.

• Work collaboratively with multidisciplinary teams and contribute to hospital discharge planning where required.

• Provide advice on community resources, preventative services, and self-management strategies.

• Maintain accurate and timely case recordings using electronic case management systems.

• Prepare reports, recommendations, and supporting documentation for funding applications and statutory processes.

• Participate in safeguarding enquiries in accordance with local authority procedures and statutory guidance.

• Contribute to service development, quality improvement initiatives, and ongoing professional development activities.

Requirements:

• Degree or equivalent qualification in Occupational Therapy.

• Current HCPC registration.

• Experience working within Adult Social Care, Community Occupational Therapy, or Rehabilitation Services.

• Strong knowledge of equipment provision, moving and handling, and home adaptations.

• Experience undertaking strengths-based assessments and person-centred care planning.

• Excellent communication, organisational, and problem-solving skills.

• Ability to work independently and effectively within multidisciplinary teams.

• Full UK driving licence and access to a vehicle may be required.
