What you will do:

Develop and implement personalised recovery plans for individuals with complex mental health needs.

Build positive therapeutic relationships with service users, supporting them throughout their recovery journey.

Work collaboratively with multidisciplinary teams and partner agencies to deliver holistic and coordinated care.

Provide practical and emotional support to help individuals work towards achieving their recovery goals.

Monitor progress regularly and adapt recovery plans and interventions as required.

Undertake assessments and contribute to risk management and support planning processes.

Promote independence, resilience, and wellbeing through strengths-based approaches.

Support service users to access healthcare, housing, employment, education, and community resources where appropriate.

Maintain accurate, timely, and confidential records in accordance with organisational policies and legal requirements.
