About the Role
Bromley’s Hospital Social Work Team is at the forefront of delivering safe, strengths-based assessment and timely discharge planning. You will partner with hospital clinicians, community services, and voluntary organisations to promote independence, reduce delays, and ensure the best possible outcomes.
As part of an integrated discharge environment, you will take responsibility for assessing needs under the Care Act 2014, coordinating discharge pathways such as Discharge to Assess (D2A) and Home First, and supporting adults with a range of health and social care challenges.
What You Will Do

Carry out Care Act assessments and design appropriate discharge plans for adults leaving hospital.

Work confidently within frameworks including Home First, D2A, and reablement pathways.

Identify, assess, and manage risks to ensure safe discharges.

Conduct safeguarding enquiries and complete Mental Capacity Assessments.

Collaborate with multidisciplinary health and social care teams to support person-centred decisions.

Manage a varied caseload, coordinating community services and post-discharge support.

Ensure legal and professional compliance across all areas of practice.
Requirements
Essential:

Qualified Social Worker with Social Work England registration.

Experience in Adult Social Care, ideally within a hospital environment.

Strong working knowledge of the Care Act, Mental Capacity Act, and hospital discharge pathways.

Ability to manage high-pressure situations and complex caseloads.

Excellent communication and multidisciplinary working skills.

Fully on-site availability—no remote working.
Desirable:

Experience within NHS or integrated care teams.

Familiarity with Discharge to Assess, Home First, and reablement models.

Understanding of CHC/DST processes.
