What you will do:
- Carry and manage a caseload of patients, collaborating with a Multi-Disciplinary Team (MDT) that includes Occupational Therapists, Physiotherapists, Speech and Language Therapists, Dieticians, and more.
- Provide expert guidance to the team on social care roles and processes, including the Care Act.
- Engage in joint sessions, family meetings, and safeguarding meetings to ensure comprehensive patient care.
- Anticipate and address issues ahead of discharge planning to ensure smooth transitions for patients.
- Foster proactive discussions with patients and families regarding discharge options and community support.
- Signpost patients and carers to additional resources such as benefits advice and housing assistance.
- Problem-solve and coordinate with external agencies to address issues like housing, homelessness, or lack of public funds.
- Contribute to managing patient flow across the inpatient pathway, ensuring high-quality care.
- Participate in data collection and analysis to evaluate the impact and success of the role.
