What you will do:
- Conduct thorough mobility assessments and develop individualised exercise programmes for patients.
- Collaborate with the healthcare team on discharge planning, ensuring seamless transitions for patients between care settings.
- Supervise and offer guidance to junior staff members, fostering a supportive learning environment.
- Facilitate the provision of equipment while utilising essential manual handling skills.
- Engage in regular home visits, providing vital physiotherapy support across Chester-le-Street / Shotley Bridge, hence a driver's licence and ability to carry equipment are a must.
- Apply your expertise to support patients with common conditions such as arthritis, heart disease, diabetes, dementia, MSK issues, COPD, and those prone to falls.
Requirements:
- HCPC registration is essential to ensure the highest standard of professional practice.
- You must be a confident car driver, comfortable with transporting equipment to various locations.
