We are seeking an exceptional Band 6 Substance Misuse Practitioner to join our team based in Guernsey. What you will do:

- Deliver specialist assessments and interventions as part of the Community Drug and Alcohol Team in various settings
- Manage the initial stages of medication induction and detoxification for individuals in both community and inpatient settings
- Coordinate and work closely with multi-agency teams and multidisciplinary partners to ensure comprehensive care and support
- Develop, implement, and evaluate tailored care programmes for clients with complex needs
- Undertake comprehensive risk assessments and implement risk management protocols
- Act as a named care coordinator, facilitating client care reviews and coordinating care package progress
- Maintain high standards of clinical record-keeping and provide comprehensive reports as required
- Contribute to the development of operational policies and clinical governance strategies
- Participate in multi-agency meetings and contribute to safeguarding procedures
- Provide guidance, advice, and professional supervision to staff and students

Minimal Qualifications and Key Criteria:

- Professional registration (RGN/RMN) or a social work qualification
- Evidence of CPD at Diploma level and related field qualification
- Experience in multidisciplinary teams and working with substance misuse clients
- Advanced skills in specialist interventions and risk assessment
- Strong communication abilities and information technology skills
