What you will do:

• Ensure that placements continue to meet the assessed needs of individuals placed and meet expectations in terms of quality of service and cost-effectiveness.
• Undertake reviews of individuals in all commissioned services, ensuring that six key elements are part of all reviews, including verifying assessed needs, capturing service user and family feedback, and arranging service changes if needed.
• Ensure that reviews are strengths-based and conducted under the guidance and supervision of the Senior Practitioner, adhering to principles of the Care Act.
• Work with relevant colleagues to develop and monitor outcomes-based support plans to maximise individual independence.
• Ensure all commissioned services meet required standards and contracted outcomes.
• Assist in the delivery of a thematic area commissioning delivery plan and contribute to ongoing market development.
