We are on the lookout for a Band 7 Pharmacist - Mental Health to join us in the vibrant city of Plymouth, UK. What you will do:

- Clinically screen medication charts on the ward to ensure accurate and effective patient care.
- Screen discharge prescriptions and one-stop orders to maintain seamless transitions for patients.
- Complete thorough medicines reconciliation and address ward-based medical information queries.
- Engage in multidisciplinary team (MDT) working to deliver comprehensive patient care.
- Conduct some clinical checking and ward-controlled drug audits to uphold safety standards.
- Order essential medicines, collaborating closely with Medicines Optimisation technicians.
- Interpret blood results in relation to medicinal treatments to optimise patient outcomes.

To succeed in this role, you must be a registered pharmacist with the General Pharmaceutical Council (GPhC). While hospital experience is preferred, it is not essential if you have been qualified and working for more than a year. An interest or experience in frailty, stroke, brain injuries, or rehabilitation, along with an understanding of polypharmacy and deprescribing, is highly advantageous. A valid HCPC registration is also required, and a driving licence is beneficial as you may need to travel between sites.
