What you will do:
- Visit people at home or in bed-based Intermediate Care to carry out Occupational Therapy assessments of their functional abilities concerning activities of daily living as part of a multi-disciplinary team.
- Undertake Community Care Assessments and collaborate with colleagues in Support Planning to deliver personalised support plans.
- Advise and support individuals and/or their carers in advancing their rehabilitation goals, offering counselling support as necessary.
- Liaise with staff of residential establishments on the rehabilitation needs of the physically disabled individuals in their care.
- Recommend equipment to increase or maintain independence and/or assist carers, and arrange its provision where necessary.
- Recommend adaptations to enhance independence and assist with the provision, ensuring completed adaptations meet standards.
- Teach the safe usage of equipment, adaptations, and alternative methods to enhance independence and support carers.
- Refer individuals to internal and external agencies for additional services as appropriate.
- Encourage rehabilitation outside the home through collaboration with Day Centres and other agencies and initiate projects to meet specific needs.
Requirements:
Candidates must hold a valid HCPC registration and a UK driving licence to effectively travel and respond to the needs of the community.
