What you will do:
- Work in an integrated partnership with the Community Mental Health Team and East London NHS Trust.

- Engage with individual service users in hospitals and community settings, offering guidance, assistance, assessments, and reviews designed to better outcomes for them and their carers.
- Creatively handle complex situations and liaise with professionals to develop effective care plans.
- Maintain high professional standards and record keeping to stay compliant with regulations.
