Key Responsibilities:

• Conduct Care Act Assessments and develop comprehensive support plans.

• Perform Mental Capacity Assessments and make best interest decisions.

• Apply a strong understanding of Deprivation of Liberty Safeguards (DoLS) and support s.21A Court of Protection work.

• Engage in Section 42 safeguarding enquiries, ensuring legal and procedural compliance.

• Collaborate with community-based adult social work teams to support people with learning disabilities.

• Use Continuing Health Care (CHC) Checklists and Decision Support Tools as appropriate.

• Carry out home visits, with a requirement to be onsite in Rotherham three days per week.

About You – Essential Requirements:

• HCPC-registered Qualified Social Worker with post-qualification experience.

• Proven background in adult social care, particularly with older adults or individuals with physical, cognitive, or learning disabilities.

• In-depth knowledge of the Care Act, DoLS, Mental Capacity Act, and safeguarding procedures.

• Skilled in s.21A court work and familiar with CHC protocols.

• Ability to work both independently and as part of a multidisciplinary team.

• Willingness and ability to travel locally for home visits.
