What you will do:

• Conduct independent assessments, treatments, and discharge planning for patients within the community rehab team.

• Engage in equipment reasoning and ordering, ensuring the right tools are available for effective patient care.

• Utilize recognisable outcome measures to track patient progress and outcomes.

• Host family meetings to discuss treatment plans, adjustments, and progress.

• Collaborate with a multi-disciplinary team, sharing insights and strategies for patient care.

• Drive to various community settings, so a valid driving licence is essential.

Requirements:
To be successful in this role, you must hold HCPC registration and ideally have experience with the NHS and post-qualification as an Occupational Therapist within community and rehabilitation settings.
