What you will do:

• Conduct complex strength-based assessments of adults, carers, and families, leading multi-disciplinary teams to deliver specialised support.
• Identify risk areas and ensure that robust safeguarding measures are in place at all times.
• Engage in supportive and preventative work with families, focusing also on areas such as substance misuse and mental health.
• Carry out visits to adults and families, ensuring their needs are met and working towards the least restrictive care solutions.
• Collaborate with families and external partners to develop comprehensive care plans.
• Lead safeguarding investigations and assist less experienced Social Workers.
• Maintain accurate and up-to-date client records and comply with data protection principles.
• Work with a variety of organisations to ensure positive outcomes for adults.
• Actively pursue personal and professional development through training and supervision.
