What you will do:
- Act as a Duty Officer, offering initial contact and support to patients outside of planned appointments, including drug testing and issuing prescriptions.
- Accept self-referrals and screen new referrals, determining priority based on eligibility criteria.
- Serve as a keyworker for patients with low complexity, engaging in multi-disciplinary assessment, treatment, and support planning.
- Support community detoxification and implement post-detox relapse prevention strategies.
- Independently make professional decisions on care delivery and risk management in various situations.
- Collaborate with the multi-disciplinary team in risk assessment and treatment modification.
- Participate in multi-disciplinary team activities and ensure the delivery of care according to individual care plans.
- Build and maintain therapeutic relationships with patients, carers, and families.
- Engage patients in activities tailored to their holistic health needs.
- Develop and deliver patient-specific interventions and negotiate to ensure compliance with other professionals and agencies.
- Operate as the Duty Officer, providing essential support and advice during crisis situations.
- Conduct Take Home Naloxone (THN) Training for patients at risk of opioid overdose.
- Screen and refer eligible cases to Tier 2 service providers, promoting coordinated care.
- Maintain accurate patient records and data entry within the electronic Health & Social Care database.
- Communicate effectively with all stakeholders in line with the Data Protection Act 2018.
- Fulfil all policy and procedural adherence requirements.
- Actively participate in health education and community service provision.
