What you will do:

Conduct statutory assessments for children with disabilities to pinpoint needs and develop tailored support plans under the Children's Act.
Design and execute comprehensive care plans for children in need including child protection and looked after children ensuring individualised support is at the forefront.
Engage directly with children and their families through person-centred approaches promoting a spirit of inclusion safety and wellbeing.
Promptly identify and address any safeguarding concerns leading child protection enquiries and interventions when required.
Foster collaboration across multi-agency partners including health education and voluntary services to seamlessly coordinate care and support.
Facilitate seamless transitions to adulthood by strategically planning with young people families and adult services.
Maintain precise and up-to-date case records documentation and reports that meet all legal and procedural requirements.
Champion the rights and best interests of disabled children ensuring their voice guides every decision made.
