What you will do:

• Conduct comprehensive assessments of older adults within the community to identify mental health, social care, and support needs.

• Develop, implement, and review personalised care and support plans in collaboration with multidisciplinary colleagues.

• Deliver therapeutic interventions and ongoing support to help individuals achieve their personal goals and maintain independence.

• Monitor and evaluate service user progress, adjusting care plans and interventions where required.

• Advocate for service users' rights, wellbeing, and access to appropriate services and resources.

• Work collaboratively with families, carers, and external agencies to strengthen support networks and improve outcomes.

• Contribute to safeguarding processes and risk management planning where appropriate.

• Participate in multidisciplinary meetings, reviews, and case discussions to ensure coordinated service delivery.

• Maintain accurate, timely, and comprehensive case records in line with organisational policies and statutory requirements.

• Promote strengths-based and person-centred approaches in all aspects of practice.
