What you will do:

  • Conduct comprehensive physiotherapy assessments for patients with acute stroke and neurological conditions, using specialist clinical knowledge to inform diagnosis and treatment planning.

  • Develop, implement, and review individualised physiotherapy treatment programmes focused on improving mobility, strength, function, and independence.

  • Monitor patient progress and adapt treatment plans as required to achieve optimal rehabilitation outcomes.

  • Manage a varied caseload, ensuring high standards of care and effective prioritisation of clinical needs.

  • Facilitate early supported discharge processes, working closely with community rehabilitation services to ensure smooth transitions of care.

  • Collaborate with multidisciplinary colleagues at team meetings to support coordinated and holistic patient care.

  • Provide education, advice, and support to patients and their families throughout the rehabilitation process.

  • Maintain accurate, detailed, and timely clinical records in accordance with professional and organisational standards.

  • Participate in clinical audits, quality improvement initiatives, and service development activities.

  • Support and supervise junior physiotherapy staff, students, and support workers as appropriate.

  • Promote evidence-based practice and contribute to the ongoing enhancement of physiotherapy services.
