What you will do:

  • Deliver high-quality mental health services that support recovery, wellbeing, and positive outcomes for service users.

  • Undertake comprehensive mental health assessments and develop person-centred care plans.

  • Provide clinical and therapeutic interventions within community settings, including service users' homes where appropriate.

  • Act as Care Coordinator or Lead Professional where required, ensuring coordinated and effective care delivery.

  • Work collaboratively with multidisciplinary colleagues and local authority services to provide integrated support.

  • Support service users to access community resources and promote social inclusion and independence.

  • Build positive relationships with service users, carers, families, and partner agencies.

  • Monitor and review care plans, adjusting interventions to meet changing needs and circumstances.

  • Maintain accurate, timely, and professional clinical records in line with trust policies and NMC standards.

  • Provide leadership, supervision, and support to colleagues and contribute to team development.

  • Participate in quality improvement initiatives and service development projects.

  • Promote positive attitudes towards mental health and advocate for the rights and wellbeing of individuals experiencing mental ill health.
