What You Will Do:

• Carry out comprehensive mental health assessments and brief interventions for service users.

• Provide short-term and long-term keyworking, supporting individuals under the Care Programme Approach (CPA).

• Manage a complex caseload, ensuring high-quality and person-centred service delivery.

• Contribute to the duty rota, ensuring seamless team operations and effective crisis response.

• Collaborate with health professionals, housing services, and community partners to deliver integrated and holistic care.

• Maintain accurate and timely case records in line with statutory and organisational standards.
