Join us for an exciting opportunity as an Adults Social Worker within the Community Team in Manchester. Based at Withington Community Hospital on Nell Lane, this hybrid locum role offers a competitive hourly rate of £30.00 and an initial three-month contract. Working Monday to Friday from 09:00 to 16:30 (35 hours per week), you will be part of an Integrated Neighbourhood Team supporting adults and families in the community. In this role, you will manage a complex caseload and work closely with health and community partners to deliver positive outcomes.

What you will do:
Carry out Care Act assessments and develop support plans to ensure individuals receive appropriate services.
Review existing care packages to ensure they remain effective and achieve the desired outcomes.
Conduct Mental Capacity assessments and make Best Interest decisions where required.
Manage risk and undertake safeguarding enquiries under Section 42 to protect vulnerable adults.
Complete home visits and collaborate with allied health professionals to provide holistic care.
Participate in duty cover approximately every six to eight weeks to support the team.

Requirements:
Qualified Social Worker with registration with Social Work England.
Experience in Adult Social Care and the ability to manage complex caseloads.
Knowledge of Liquidlogic Adults system is beneficial but not essential as training can be provided.
