What you will do:

  • Provide holistic support through Islington's Take Home and Settle Service, ensuring smooth and safe transitions from hospital to home.

  • Undertake initial home assessments to evaluate safety, wellbeing, and recovery needs following discharge.

  • Carry out statutory responsibilities in accordance with the Care Act 2014 and Mental Capacity Act 2005.

  • Work collaboratively with residents, carers, families, and community organisations to maximise independence and minimise the need for ongoing interventions.

  • Use strengths-based and person-centred approaches to identify and build upon individual strengths and support networks.

  • Undertake Mental Capacity Assessments and coordinate care arrangements where required.

  • Maintain accurate, timely, and professional case records, ensuring compliance with data protection and information governance requirements.

  • Provide specialist advice and guidance to NHS colleagues, community organisations, carers, and other stakeholders.

  • Escalate complex issues and significant developments to senior practitioners appropriately.

  • Promote high standards of health and safety, customer service, and professional practice.

  • Work effectively within a multidisciplinary team to ensure coordinated and outcome-focused support planning.
