What you will do
- Work in a multi-agency context, sharing information, conducting joint assessments, and implementing care planning to provide a comprehensive understanding of needs.
- Promote and safeguard the welfare of vulnerable adults, ensuring their safety and well-being.
- Manage a caseload with varying degrees of complexity, serving as the care coordinator for assigned cases.
- Conduct Mental Capacity Assessments in line with the Mental Capacity Act 2005.
- Ensure the implementation of the Care Act 2014 in all aspects of your role.
- Work collaboratively with service users, carers, and other professionals to minimise and manage risk while maintaining preferred lifestyles.
- Obtain informed consent for all care-coordinated cases and keep senior practitioners informed of significant developments.
- Provide expert advice and consultancy to colleagues and stakeholders, including the NHS, the Independent sector, and the public.
- Maintain accurate records and enter data efficiently as per Council policy, attending and contributing to multi-disciplinary meetings.
- Engage in regular supervision and training sessions to further develop your practice and achieve agreed service outcomes.
- Uphold the Council’s commitment to customer service excellence and ensure compliance with customer care standards.
- Comply with relevant legislation, including the Data Protection Act and Health and Safety policies, to ensure a safe working environment for all.
