What you will do
- Conduct Care Act 2014 assessments, reviews, and support planning focused on outcomes for adults aged 18 and over.
- Manage a varied caseload involving adults with complex physical health issues, learning disabilities, mental health challenges, and age-related support needs.
- Undertake safeguarding enquiries and risk assessments following local safeguarding procedures and Making Safeguarding Personal principles.
- Collaborate with NHS colleagues, occupational therapists, community health teams, housing providers, and voluntary sector partners within a multidisciplinary framework.
Person Specification
- Qualifications: A Degree in Social Work, DipSW, CQSW, or equivalent recognised social work qualification is essential. Demonstrated commitment to ongoing Continuing Professional Development (CPD) is crucial. A Best Interests Assessor qualification is desirable.
- Registration & Compliance: Must be registered with Social Work England (SWE).
- Experience: Previous experience in a statutory Adult Social Care environment is required, along with a proven track record of supporting adults with complex care and support needs.
