What you will do
- Collaborate within a multi-agency framework to provide holistic care planning.
- Safeguard the welfare of vulnerable adults, ensuring their safety and well-being.
- Manage a diverse and complex caseload, handling all care coordination duties.
- Conduct Mental Capacity Assessments and enforce the Care Act 2014 requirements.
- Foster independence through relationship-based and person-centred approaches.
- Inform senior practitioners of significant events and coordinate service operations efficiently.
- Offer expert advice to colleagues and stakeholders within and outside the NHS.
- Maintain comprehensive records as per Council Policy and Data Protection Laws.
- Attend disciplinary meetings and achieve service goals through effective communication.
- Ensure compliance with the Customer Service Excellence and Health and Safety standards.
- Use social work methods to address personal and social challenges faced by clients.
