What you will do:

Undertake Care Act 2014 assessments within a hospital setting to support safe and timely discharge.

Facilitate discharge planning in line with Homefirst/Discharge to Assess (D2A) pathways, promoting independence and recovery.

Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.

Work collaboratively with health professionals, discharge coordinators, families, and community services to coordinate care packages.
Person Specification:
-
Degree in Social Work (BA/MA or equivalent)

Evidence of continued professional development

Registered with Social Work England

Knowledge of relevant legislation and frameworks, including the Care Act 2014, Mental Capacity Act 2005, and hospital discharge guidance.

Enhanced DBS clearance
