What you will do:

• Undertake Care Act 2014 assessments within a hospital setting to support safe and timely discharge.

• Facilitate discharge planning in line with Homefirst/Discharge to Assess (D2A) pathways, promoting independence and recovery.

• Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005.

• Work collaboratively with health professionals, discharge coordinators, families, and community services to coordinate care packages.

Person Specification:

• Degree in Social Work (BA/MA or equivalent)

• Evidence of continued professional development

• Registered with Social Work England

• Knowledge of relevant legislation and frameworks, including the Care Act 2014, Mental Capacity Act 2005, and hospital discharge guidance.

• Enhanced DBS clearance
