What you will do:

  • Undertake comprehensive mental health and substance misuse assessments, including risk assessments and care planning.

  • Act as Care Coordinator under the Care Programme Approach (CPA), managing a caseload of service users with dual diagnosis.

  • Deliver evidence-based interventions, including relapse prevention, psychoeducation, and harm reduction strategies.

  • Support and monitor medication management and prescribing plans in collaboration with medical staff.

Person Specification:

Qualifications:

  • Registered Mental Health Nurse (RMN)

  • Evidence of continued professional development

Registration & Compliance:

  • Active registration with the Nursing and Midwifery Council (NMC PIN)

  • Knowledge of relevant legislation and frameworks:

  • Mental Health Act 1983/2007

  • Mental Capacity Act 2005

  • Care Programme Approach (CPA)

  • Enhanced DBS clearance required
