What you will do:

Deliver a specialist community Occupational Therapy service to disabled individuals across age groups with complex health, social, psychological, and housing needs.

Independently manage a dynamic caseload as an autonomous practitioner, using advanced problem-solving, clinical reasoning, and professional judgement.

Implement relevant legislation and adhere to organisational policies, health promotion strategies, and RCOT professional standards.

Assess the interaction between health, housing, and social factors, interpreting technical plans and advising on housing adaptations and Disabled Facilities Grants.

Provide advice and support to individuals, families, and carers, including demonstrating techniques and equipment to enhance independence.

Organise the provision of standard and specialist disability equipment with cost-effective decision-making.

Conduct person-centred assessments, respecting individual perspectives, wishes, and cultural needs while collaborating with partner agencies.

Maintain accurate and timely records in line with legal and organisational requirements.

Carry out complex risk assessments and provide moving and handling guidance to external agencies where appropriate.
